Community PACE was designed to provide all medical and social services that allow the elderly population to remain safely in the community for as long as possible. Our team includes Primary Care Providers, Nurses, CNAs, Social Workers, Homecare Coordinator, Physical Therapy, Occupational Therapy, Transportaion, Dietary Services, Recreational Therapy, Case Management, and many other supporting staff members.
