Element Care provides comprehensive healthcare and social services to adults 55 and older through the Program of All-inclusive Care for the Elderly (PACE). The program helps seniors live safely, comfortably and well at home and in their communities for as long as they can while minimizing stays in hospitals and nursing facilities.
