Norwalk Community Health Center is a Patient-Centered Medical Home where the focus is on you! In a state-of-the-art medical facility on 120 Connecticut Avenue in Norwalk, and aboard our traveling Mobile Medical Unit, our patients have access to comprehensive adult medicine, pediatric medicine, women’s health (OB/GYN), behavioral health, and pediatric/adult dental services and wellness programs. NCHC is a non-profit 501(C)3, independent healthcare organization dedicated to leading our community toward the achievement of the best possible health.
